Report without putting your name to it
A near miss is only reported when reporting costs nobody anything. So the CIRS table in Viali has no author column. Not hidden, not encrypted, not protected by a setting — it does not exist.

There is no author column for anybody to read
Most reporting systems promise anonymity and store the author anyway — just in case, for the follow-up question, for later. That «for later» is exactly why nobody reports. In Viali the table holds a title, the text, a status and a timestamp. No user id, no name, no IP address. Open the database and there is nothing to search on. This is not a setting an administrator can change.
Two fields, in the same panel as support and SOPs
A report is a title and a description. Nothing more is asked, because every extra required field raises the barrier and makes the report easier to attribute. Anyone signed in at the clinic can file one — nursing, anaesthesia, surgeons, reception. The form sits in the help panel beside support and SOPs, where people already look things up, rather than in a module you have to go and find.
New, reviewed, closed — and only the leadership reads them
Reports arrive in one list under Admin → Clinical → CIRS, visible to the clinic leadership only. Each has three states: new, reviewed, closed. The counter at the top shows how many sit in each — a growing number under «new» is the only metric this module really needs. What follows from a report is the clinic's decision: a task on the board, a new SOP, a change to how something is stored.
How many near misses did your clinic write down last year?
If the answer is none, that rarely means none happened. Try Viali free for 14 days, or request a demonstration — we will walk from the anonymous report to the SOP that comes out of it.
